Jayla Monique Collins was born in Pittsburgh, Pennsylvania on December 20, 2008 to Jon Douglas Collins Jr. and Monique Ashley Caldwell. Jayla was presented to the Lord in Dedication on March 8, 2009 at the Mount Horran Baptist Church by Pastor Willie E. Bishop.

Jayla peacefully returned to the Lord on April 24, 2009.  She leaves to cherish her memories, her father Jon Douglas Collins Jr., her mother Monique Ashley Caldwell, her brother Jamir Jon Collins, Grandparents Thomas and Donna Boyd and Edward and Susan Diane Caldwell, Great-Grandparents John and Jackie Collins, Deborah Smith and Rose Thompson, and a host of uncles, aunts, cousins and relatives.  She was recently preceded in death by her Great Grandfather Donald Clancy and Harold Thompson and Great Uncle Curtis Smith.
